2 weeks (14 days) after your second dose of an accepted 2-dose series
2 weeks (14 days) after you received the full series of an accepted COVID-19 vaccine (not placebo) in a clinical trial
2 weeks (14 days) after you received 2 doses of any "mix-and-match" combination of accepted COVID-19 vaccines administered at least 17 days apart*

When will the USA end pre-departure testing?
Still no answer to this. There were rumours it would be mid-May, still nothing yet. There has not been any news, and gossip which does not serve any purpose has quietened down. So no news as of yet but as soon as I know I will publish on the blog.
On the CDC attestation form, do you need to include middle names to match your passport? The form specifically says first and last name so not 100% sure what to put!
