UPDATED: Visiting the US soon? How to complete the new US Attestation and Passenger Disclosure Form.

Here is also the link to the new, updated Attestation Form. You can choose English or whatever your language.  


It's simpler and quicker to complete. However do yourself a favour, print it, complete it, sign in and bring to the airport. 

As the form says, one attestation form must be filled out for each passenger, age 2 years or older who is not a U.S. citizen, U.S. national, lawful permanent resident, or an immigrant ("Covered Individual"), and who is seeking to enter the United States by air travel.

The attestation may be filled out by the air passenger or on behalf of an air passenger by a legal representative, such as a parent or guardian. The passenger must also be able to check all boxes related to and comply with applicable after travel requirements to board a plane to the United States.

Under the first section you fully print your name,  and indicate that you are either acting upon your own behalf, or on behalf of someone else. 

Section A, declare you are fully vaccinated. 

If you don't have children, it's just yourself then tick you are fully vaccinated then you can then go directly to the signature section to complete the Attestation Form. That's it, the end. 

Section B if your children are neither vaccinated or not fully vaccinated. 

Or Section B, not fully vaccinated or unwilling to prove proof of vaccination. You will need to read through the exemptions and select what is applicable. However one of the selections is a child aged 2 through 17 years. So select this if the child is not fully vaccinated then go to section D to complete, then sign the form to complete Attestation). 

If you're a Diplomat and Official Foreign Government Travel you complete Section C. 

IMPORTANT READ IF YOU HAVE CHILDREN:
Section D, exceptions for children between 2 through 17. 

Now Section D applies if you have children and this is the section you tick if your Child is aged between 2 through 17 years and are either not vaccinated or not fully vaccinated. 

You also agree that children are to be tested with a COVID-19 viral test 3-5 days after arriving in the United States, unless there is documentation of having recovered from COVID-19 in the past 90 days - It's very difficult to police this and I'd imagine not a lot if people will comply with this. 

Section E - medical contraindication
If you have a medical contraindication to an accepted COVID-19 vaccine as determined by CDC then complete Section E.  

Section F is Humanitarian or emergency exceptions. 

Section G is for person[s] whose entry is in the U.S. National Interest. 

FINALLY: 

Make sure to complete the Attestation Form by printing your name, signing and dating it. Print it and bring to the airport.
